Penacook Place’s short-term rehabilitation program is designed for patients who require therapy and care as they recover from illness, accident or surgery during the transition between hospital and home.
As part of our program, members of our highly trained post-acute care staff work with each patient to develop a personalized plan for recovery that focuses on the skills and techniques the patient needs to achieve independence when they return to their home. Patients in our program receive the education, therapy, emotional support and planning they need to be successful once they are ready to return home.
