Short-Term Rehab

Penacook Place offers short-term rehab and a comprehensive range of short-term services for patients who are in need of rehabilitation and nursing care while transitioning from the hospital to home or other level of care. Whatever the need, our mission is to help restore function to the maximum possible level and “to care for our patients and residents as we would want to be cared for.”

Penacook Place's rehabilitation team is focused on the unique concerns of our individual patients. We know that following an injury, illness, or hospitalization, our patients want to return home and regain their independence as quickly as possible. Whether it is orthopedic trauma such as joint replacements or fractures, falls, strokes, myocardial infarctions, pneumonia, or other issues, Penaccok Place has specialized rehabilitation services available 6/7 days a week to meet individual needs and promote recovery, including:

Physical therapy Occupational therapy Speech therapy Home evaluations	Pain management Orthopedic care Wound management Cardiac care	Activities of daily living training Family and caregiver teaching IV therapy Nutritional services
Social service, including discharge planning

Our physical therapists, occupational therapists, speech and language therapists, and other rehab specialists have an average of 16 years clinical experience with a focus on acute care, long-term care, home care, wound care, orthopedics, chronic care, geriatrics, and outpatient care. Weekend coverage is available.
